2025 – 2026 Application Guidelines & Scholarship Details
Every year The Guild of the Children’s Diabetes Foundation (CDF) provides Charlotte Tucker Scholarships to type 1 patients of the Barbara Davis Center (BDC). These scholarships are primarily based upon financial need, with all other aspects of the application taken into consideration. Scholarships may be used for an accredited 2- or 4-year college/university, trade school, or vocational school for the 2024-2025 school year. Scholarships are not available for graduate or specialty programs beyond a bachelor’s degree.

Qualifications
- Be a current BDC patient diagnosed with type 1 diabetes
- Have been seen at the BDC in the last 12 months
- Have a minimum cumulative 2.0 GPA
- Plan to or currently attend a 2-year or 4-year college or trade school with a minimum of 12 semester hours or equivalent
- Commit to completing a minimum of 8 hours of volunteer service by the summer following your 2026 spring semester, if awarded a scholarship

All applicants (new and returning) are required to submit a new application packet.
PLEASE SEND ALL MATERIALS IN ONE APPLICATION PACKET
- The application form filled our completely and typed in editable PDF application form or legibly handwritten. Do not leave questions unanswered. Signatures must be handwritten.
- An official high school or college transcript is required, showing at least 1 1/2 years of academic performance. Both high school and college are required if you are just completing your first year of college.
- Your ONE PAGE resume, including information about your involvement in volunteerism, leadership, clubs, sports and activities. Include any achievements and/or awards received and your long-term goals. Please also describe how receiving this scholarship will help you achieve these goals.
- Two (2) evaluation forms are recommended, filled out completely and legibly (typed in editable PDF application form or handwritten.) No relatives or peers may fill out an evaluation form. One evaluation form must come from a current teacher if possible. If you are in home school or only online, one evaluation must come from an outside educator in the home school/online process, clergy, scout leader or leader of volunteer organization you are involved in. Follow up with those who have filled out your evaluation forms and forward with the rest of your application if possible. - A personal statement, 300-500 words (typed, double spaced) written by scholarship applicant. This must accompany your application materials! The “Personal Statement’ portion of your application is one of the more important parts of your packet. Have someone edit your statement as a poorly written or edited statement is a reflection of you. Your application will not be considered if it does not contain your Personal Statement! You are 100% responsible for turning in a well-written Personal Statement. Your statement is best written when you tell a story. This story may include the triumphs and challenges you have encountered in your life and as a student with Type 1, but it doesn’t have to. Whatever story you choose should be representative of you as a unique individual.
Tips for Applying for Scholarships
- Read through the entire application first before starting to fill it out. Don’t jump in without knowing what is fully required for the application.
- Allow for additional time for your evaluation forms and transcripts. Schools and teachers are fielding requests from many students at the same time you are asking for your transcripts and evaluation forms and will need ample time to turn in the requested materials.
- Quality recommendations are of utmost importance, so ask someone who really knows you and include things that make you stand out such as your volunteer services, Eagle Scouts, extracurricular activities, working history, etc.
- A resume is a BRIEF document that summarizes your education, employment history, volunteer history, and relevant experiences. Your resume needs to be user-centered and persuasive without being too long. A general-purpose resume usually contains four sections: Contact, Education, Experience, and Honors, Activities, & Outreach.
WHEN MAILING YOUR APPLICATION MATERIALS DO NOT STAPLE ANY OF THE APPLICATION PAGES TOGETHER. ALL PAPERS SHOULD BE ONE-SIDED AND FREE OF STAPLES, CLIPS OR OTHER BINDING.
